Passport application registration Form (total 1 Page)
PASSPORT APPLICATION FORM (FORM No. 1) total 2 copy required (4 pages X 2 = 8 pages)
PERSONAL PARTICULARS FORM total 2 copy required (2 page X 2 = 4 pages) Fill the above forms and have following documents (Original and 2 copies) 1. Address proof for full one year 2. Age proof Following additional documents required: 1. Marriage certificate if want to include spouse name in Passport and 2 copy 2. old expired passport and 2 copy if applying for re-issue
Proof of urgency if want in Tatkal category 4. 1. Reach passport office at about 9:00 am and stand in a big queue which ends outside passport office boundary. Alternatively you can get the appointment on internet and go at appropriate time (Normally from 2:30 pm) 2. A staff will put a token number sticker on your form at the end of the above queue. 3. Stand in another queue on the first floor.
One staff will verify all the papers and documents. Then wait for token number to be announced 5. Once your name is called, go and submit the form. Here you will be asked to show the original documents which will be returned immediately. Finally pay the fee and collect the receipt.
