Just went through a similar ordeal yesday advising me a MRI for same reason what you had. I accepted their demand and ended up as a laughing stock with other hospital while re-checking.

I asked the bank sales team who replied that DB had paid price for checked fares which is untrue. I had to send a screen shot showing it was only for spcial fares which price is only 250 rupees one way!
Tickets were part of welcome gift. When i took the card prices were high but when i paid the money to bank prices had come down. I never said bank is responsible for price fall. Titanium card and standard chartered bank is the worst buch among these credit card characters. They billed me penalty even before giving first monthly statement.
